Community First Choice


For Maryland residents of all ages, living in their home, in need of support with activities of daily living, such as bathing, grooming, dressing, and accessing the community.

  • Community First Choice pays for certain services, like personal assistance, meals, and emergency back-up systems that support individuals in their home.
  • Individuals must meet an institutional level of care based on a medical assessment completed by the local health department.
  • An individual’s income and assets are reviewed to determine financial eligibility for community Medicaid.
